MtF Private Recommendations?

I have been on the NHS waitlist several times for multiple years and rejected because I have a fairly severe underlying health condition. From my perspective, though, if I'm going to be terminal, I'd much prefer to at least die happy and comfortable in my body for the first time. As such, I am seeking private MtF treatment. I have a limited budget but I would very seriously be prepared to sell everything I own if it meant that this would work for me.

I would like to have this done in London but I am prepared to travel if necessary for surgery or even an initial hormone prescription. What I am not prepared to do is spend months and years waiting. I would have the surgery done tomorrow if someone would perform it and I would happily pay my entire salary for the rest of my life to whoever would perform it. I have been fighting the NHS on this for over a decade and I am truly beyond tired of waiting.

I would appreciate any help that anyone could give me in terms of who to go to to have treatment in the most expedited manner possible and the fastest possible route to MtF genital surgery. Cost is prohibitive but, as I said, I will find a way to make it work. Location is preferably London but if someone were prepared to perform the surgery quickly and give me the hormone treatments indefinitely and it meant a flight to Turkey or Dubai or Moscow every few months, I'd do it willingly.

I am a doctor. I understand the risks of what we're talking about. I don't need to be told to be calm or be slow or that this is a process that takes time. It very well may take time but it doesn't have to. There's no medical reason why I can't be transformed and happy tomorrow except that nobody will perform the surgery. So I see no reason why I should continue to live my life completely on hold, knowing that I may only have another year or two to live, having waited my whole life to begin as a woman, when there is no reason except the assumed norm for it. If you don't agree, that's fine. I'm really only asking for advice as to where I can find this type of help. But please, please, please respect my decision on this. I spend most days of my life having to defend being trans. I don't want to have to do it online, too.
